There are three steps to organising admission to Maurice Zeffert Home.
Step 1: Aged Care Assessment
Before Maurice Zeffert Home can admit any Resident, we are required by law to obtain a detailed report from the Department of Health Ageing and Aged Care multi-disciplinary Aged Care Assessment Team (ACAT) in their area. This report is compiled by an ACAT Assessor in the home environment of the prospective resident and may take up to eight weeks to finalise, depending upon how busy their local ACAT Team may be. This assessment may be organised by contacting 1800 200 422 or through your family GP. For more information, you can visit the MyAgedCare website at www.myagedcare.gov.au
Step 2: Application for Admission
Contact reception on (08) 9375 4600 to request an Application Pack.

Step 3: Meeting to Arrange Admission
On receipt of these documents, our ACFI & Admission Coordinator will contact you when a suitable vacancy becomes available to arrange a private meeting to discuss matters relating to the admission of the prospective resident.
